By views. I check how the video does next to your other ones. A high click rate on its own doesn't mean much — a video with 5M views beats one with 100 views, even if the small one has a higher click rate.
Yes, if you ever need one — but it almost never happens. I keep making changes until you're happy. And if it's really not working out, we'll find a fair fix.
